Completing the Official Meat Inspection Certificate forms for meat products from the United States

Please note the following clarifications regarding box 2 of the Official Meat Inspection Certificate forms.

For all meat products from the United States except natural intestinal casings

Ensure that the name and address of the holder of the Safe Food for Canadians licence to import is entered in box 2 (consignee/importer) of the Certificate for Export of Meat and Poultry Products (Food Safety and Inspection Service (FSIS) Form 9135-3).

For natural intestinal casings from the United States and natural intestinal casings of foreign origin, imported into Canada from the United States

For the following forms:

Animal Casings Export Certificate for Countries Requiring Ante-mortem, Post-mortem and Fit for Human Food Statements form (FSIS Form 9060-7)
Animal Casings Export Certificate for Countries Requiring Ante-mortem, Post-mortem and Sound and Clean Statements form (FSIS Form 9060-18)
Ensure that the name and address of the holder of the Safe Food for Canadians licence to import is entered in the field marked “consigned to”.
